Please make sure to bring with you a valid insurance card; if you do not have your insurance card available, please make sure to have your policy and group numbers available for eligibility purposes. Our front-office staff will verify your eligibility for services, deductibles, co-pays, and co-insurance. Co-payments are required at time registration.
For our patient's convenience, we offer 20% discount off our cash prices.
Car Insurance for Car Accidents
For Car and Motor Vehicle Accidents, AvantGarde Urgent Care only requires you to bring your car insurance policy, claim number and date of car accident. If you have a police report we also encourage you to bring that when you come in to see our Doctors.
Workers' Compensation Insurance
We welcome Federal and Florida workers' compensation medical plans.

"I haven't been feeling so well for about 4 days. Decided to go to this urgent care facility. Came in on a Sunday morning. Two people were working and were very nice. The place was clean, but not cold as a doctor's office should be. Got me in maybe 5 mins after I filled out the paperwork. Once I was in the patient room, Dr. Malczak saw me. Nice guy. He met with me for about 20 mins discussing my symptoms and what I possibly have. Very knowledgeable and understood what I was going through. Overall, experience was great at this facility."

"My wife sliced her left index finger deeply severing an artery so it was bleeding badly. Went there an hour before closing and received prompt and courteous service.
